29 CFR 1910.1048(h)(1)(i): All contact of the eyes and skin with liquids containing 1 percent or more formaldehyde was not prevented by the use of chemical protective clothing made of material impervious to formaldehyde and the use of other personal protective equipment such as goggles and face shields, as appropriate to the operation: On or about March 20, 2025, employees working with hazardous chemicals in the Grossing Area of the Histology and Pathology Laboratory were exposed to eye and face injuries because the employer has not ensured employees wear eye and face protection to protect against formalin splashes while dumping formalin in carboy.

29 CFR 1910.1048(m)(2)(ii): As a minimum, for all materials listed in paragraph (m)(1)(i) and (iv) of this section capable of releasing formaldehyde at levels of 0.1 ppm to 0.5 ppm, labels shall identify that the product contains formaldehyde; list the name and address of the responsible party; and state that physical and health hazard information is readily available from the employer and from safety data sheets: On or about March 20, 2025, employees working with hazardous chemicals in the Grossing Area of the Histology and Pathology Laboratory were exposed to injury because the employer had not ensured the formalin carboy waste is labeled hazardous waste.
