29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Outside press - On or about March 21, 2025, the employer instructed employees to use the 300-ton full revolution press break to press a slight curve in different sizes of steel plates without having the point of operation guarded exposing employees to amputation hazards.

29 CFR 1910.217(b)(4)(ii): Mechanical power press(es) using full revolution clutches were not provided with a pad with a nonslip contact area firmly attached to the pedal: (a) Outside building, north side - On or about March 21, 2025, the employer required employees to use the foot pedal to operate the full revolution press without a pad under it to prevent the pedal from moving, exposing employees to amputation hazards.

29 CFR 1910.305(j)(4)(ii): As it applies to motors, motor circuits and controllers; a disconnecting means for the controller was not located within sight of the controller location ( i.e. that one piece of equipment shall be within sight of another piece of equipment, the piece of equipment shall be visible and not more than 50 feet from the other). (a) North Outside press break - On or about March 21, 2025, the employees operate a 300-ton full revolution press break that has the motor disconnect located inside the building out of sight, exposing employees to caught in hazards.
