29 CFR 1910.212(a)(1): One or more methods of machine guarding were not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about March 21, 2025, employees operating extrusion line 311HLF001 were not protected from ingoing nip points in that the ends of the compression rollers were not guarded. (b) On or about March 21, 2025, employees operating extrusion line 311HLF002 were not protected from ingoing nip points in that the ends of the compression rollers were not guarded. (c) On or about March 21, 2025, employees operating extrusion line 306 were not protected from ingoing nip points in that the ends of the compression rollers on the left side of the machine were not guarded. (d) On or about March 21, 2025, employees operating extrusion line 306 were not protected from ingoing nip points in that the ends of the compression rollers on the right side of the machine were not guarded. (e) On or about March 21, 2025, employees operating extrusion line 302 were not protected from ingoing nip points in that the ends of the compression rollers on one side of the machine were not guarded. (f) On or about March 21, 2025, employees operating extrusion line 312 were not protected from ingoing nip points in that the nip rollers of the winder 312WND001 were not guarded. (g) On or about March 21, 2025, employees operating extrusion line 312 were not protected from ingoing nip points in that the nip rollers of the winder 312WND002 were not guarded.
