On or about March 20th 2025  the employer failed to ensure that employees performing cleaning of the 275 gallon totes containing flammable liquids are provided with adequate protections such as but not limited to grounding cables, training, and  personal protective equipment to prevent the ignition of flammable vapors from static discharge. On or about March 20th 2025 the employer failed to ensure that containers of class 1, 2 and 3 flammable liquids, used to manufacture automotive care products, were bonded and grounded to prevent static electricity buildup during dispensing operations which can ignite flammable vapors.
