Located at the job site at 161 Ziegler Avenue, Columbus, Ohio: On or about March 24, 2025, the employer did not initiate and maintain safety and health programs to protect employees performing roofing work on an existing single-story residential home. As a result, employees were exposed to fall hazards of over 10 feet as a result.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. a. Located at the job site at 161 Ziegler Avenue, Columbus, Ohio: On or about March 24, 2025, the employer failed to ensure the employees were protected from falling when installing roofing material on an existing single-story residential home. As a result, employees were exposed to a fall of approximately 10 feet 5 inches from the roofs edge to the ground below.
