29 CFR  1926.405(b)(2):Covers and canopies. All pull boxes, junction boxes, and fittings shall be provided with covers. If metal covers are used, they shall be grounded. In energized installations each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a.)On or about March 24, 2025, the employer failed to ensure that energized junction boxes located on the ground level and the second level of a gut-rehab building were provided with covers. As a result, employees installing new wiring and making electrical connections were exposed to 208-volt electrocution hazards.

29 CFR  1926.416(a)(1): No employer shall permit an employee to work in such proximity to any part of an electric power circuit that the employee could contact the electric power circuit in the course of work, unless the employee is protected against electric shock by deenergizing the circuit and grounding it or by guarding it effectively by insulation or other means.    a.) On or about March 24, 2025, the employer failed to ensure that employees working in proximity to  live electrical junction box were protected from electric shock by deenergizing and grounding the circuit, or by effectively guarding it through insulation or maintaining a safe working distance. As a result, employees were exposed to electric shock hazards.

As a result, employees installing new wiring and making electrical connections were exposed to electrocution hazards.

29 CFR  1926.960(g)(1): Hazard assessment. The employer shall assess the workplace to identify employees exposed to hazards from flames or from electric arcs.   a.) On or about March 24, 2025, the employer failed to conduct a hazard assessment of the workplace and identify missing covers and canopies for live-active junction boxes located on the ground level, and the second level of a gut-rehab building, live electrical junction boxes were deenergized and grounded or guarded effectively by insulation or by ensuring employees worked at a safe proximity ,   ,in that the employer exposed employees to electrocution hazards of 208V, from power coming into the building from outside pole, as employees installed new wiring, and connections within a new junction box.

29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.   a) On or about March 24, 2025, the employer failed to conduct frequent and regular inspections of the jobsite by a competent person to identify hazards, including missing covers on energized junction boxes and the lack of guarding or deenergization of live electrical components. As a result, employees working on the ground and second levels of a gut-rehab building were exposed to 208-volt electrocution hazards while installing new wiring and making electrical connections in energized junction boxes.
