On or about March 12, 2025, the employer did not ensure inspections of excavations were conducted by a competent person prior to allowing employees to enter and work from trenches and throughout their shift.

On or about March 12 , the employee entered a trench with, previously disturbed soil without the use of an adequate protective system exposing that employee to a cave in.

On or about March 12, a TD Williamson employee used a portable ladder to access a trench where the ladder's side rails did not extend at least 3 feet (.9 m) above the top of the trench.
