29 CFR 1910.147(c)(4)(ii)(B): The energy control procedure did not clearly and specifically specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: On or about March 21, 2025, General Motors Corporation, located at 1500 East Route A in Wentzville, Missouri, did not follow its LOTO program, wherein a specific procedure for shutting down, isolating, blocking, and securing the equipment to control the potential residual hazardous energy. The first shift repair crew did not reinspect or reevaluate the potential residual energy that may prevent the door from being activated, resulting in the finger amputation of employee #1 from being caught between the sprocket and chain of the door mechanism when it accidentally activated.

1910.147(c)(7)(iii)(B) Additional retraining shall also be conducted whenever a periodic inspection under paragraph (c)(6) of this section reveals, or whenever the employer has reason to believe that there are deviations from or inadequacies in the employee's knowledge or use of the energy control procedures. On or about March 21, 2025, General Motors Corporation, located at 1500 East Route A in Wentzville, Missouri, did not follow its LOTO program when the door to the robotic room was repaired; The incoming repair crew did not check or reevaluate all energy sources to include the counterbalance and pole chains that were initially applied by the previous crew. The residual potential stored energy from the counterweight/gravitational pull was not rendered safe or contained before continuing with the door repair, resulting in the finger amputation of employee #1 from being caught between the sprocket and chain of the door mechanism when it accidentally activated.
