29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. At a jobsite located at 5400 Rio Grande Avenue, Jacksonville, FL: On or about March 26, 2025, the employer exposed employees to an amputation hazard, in that the there was no guarding during the manufacturing assembly step for the clevis pin installation connecting the hydraulic cylinder piston and the baler ram plate.
