On a new residential construction, at a temporally site located at 357 Bedford Terrace, Saint Augustine, FL 32092: On or about March 25, 2025, the employer exposed employees to fall hazards, in that an employee was working on a job-made Wood Pole-type Scaffold, built with 2x6 non-scaffold construction-grade wood, and was not protected from falling to that lower level.
