On or about March 25, 2025, and at times prior, the employer allowed his employees to perform roofing activities without first ensuring his crew was trained on the hazards associated with falls including, but not limited to, setting up and inspecting fall protection exposing his employees to a fall hazard of 14 feet 1 inches to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder: a) 4622 Theodore Dawes Rd. Theodore, AL. On or about March 25, 2025, and at times prior, The employer failed to ensure employees were using the ladder in a safe manor, the employer allowed employees to used the ladder to access the roof at a height of 14 feet 1 inch without ensuring the ladder extend three feet above the landing which exposed employees to a fall hazards.
