29 CFR 1910.95(c)(1):  The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  a) 39 Riverside Drive Reading, PA 19605 - Pallet Wood Shop - On or about April 16, 2025, the employer did not institute a continuing and effective hearing conservation program when employee's noise exposure exceeded an 8 hour time weighted average sound level of 85 dBA.  An employee working within the pallet processing area was exposed to noise at a level of 160% of the allowable limit during a 442 minute sampling period on April 16, 2025; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 93.3 dBA for the dose and time of the employee exposure.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR  1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k). The employer did not institute a training program and ensure employee participation in the program:   a)  39 Riverside Drive Reading, PA 19605 - Pallet Wood Shop - On or about April 16, 2025, the employer did not train all employees who were exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k)(3).  An employee working within the pallet processing area was exposed to noise at a level of 160% of the allowable limit during a 442 minute sampling period on April 16, 2025; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 93.3 dBA for the dose and time of the employee exposure.  Abatement certification required within 10 days after abatement date. 29 CFR 1910.95(g)(1):  The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:   a) 39 Riverside Drive Reading, PA 19605 - Pallet Wood Shop - On or about April 16, 2025, the employer did not establish and maintain audiometric testing for employees with exposures that equaled or exceeding an 8-hour time-weighted average of 85 decibels. 29 CFR 1910.95(d)(1):  When information indicated that any employee's exposure equaled or exceed the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program:   a) 39 Riverside Drive Reading, PA 19605 - Pallet Wood Shop - On or about April 16, 2025, the employer did not institute a continuing and effective hearing conservation program when employees' noise exposure exceeded an 8 hour time weighted average sound level of 85 dBA.  An employee working within the pallet processing area was exposed to noise at a level of 160% of the allowable limit during a 442 minute sampling period on April 16, 2025; exposure calculations included a zero increment for the 30 minutes not sampled. This exposure is equivalent to a sound level of 93.3 dBA for the dose and time of the employee exposure.  Hannahoe's Pallets and Skids, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.95(d)(1), which was contained in OSHA inspection number 1782056, citation number 1, item number 1 and was affirmed as a final order on January 7, 2025, with respect to a workplace located at 39 Riverside Drive, Reading, PA 19605.  No abatement certification or documentation required.
