At the worksite, located near Paseo Al Mar Blvd. and US Highway 301, Apollo Beach, FL 34219; On or about 3/25/25, the employer exposed employees to struck by and crushing hazards in that a suitable means of access/egress was not provided to employees working in an unprotected excavation. Employees entered and exited the excavation by climbing the excavation edge while installing a storm drain line within an excavation approximately 11 feet in depth.
