29 CFR  1910.146(c)(2):The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:(a)(LOCATION)(IDENTIFY SPECIFIC OPERATIONS AND/OR CONDITIONS)NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign.  a. On or about March 25, 2025, at 4440 Chisholm Road, Florence, AL, at the Muller mixer, the employer did not have notices alerting employees accessing the 3G Simpson Mix-Muller machine of confined spaces  with signage containing "DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER" or using other similar language.  Imperial Casting Company, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.146(c)(1):, which was contained in OSHA inspection number 1545451, citation number 1, item number 2 and was affirmed as a final order on December 09, 2022, with respect to a workplace located at 4440 Chisholm Road, Florence, AL.

On or about March 25, 2025, at the Simpson 3G Muller Mixer, employee(s) were required to collect a sample of the product after each batch is mixed by sticking their hand inside the machine while it was still operating and without the use of an adequate hand tool for the task.
