29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about March 18, 2025, Primoris Renewable Energy, located at Kelso Solar Project, 5351 Hwy H, in Benton, Missouri did not instruct the Skid Steer Operator to recognize any hazard associated with the blind spots associated with the operation of this equipment resulting to hitting and running over a ground crew causing a broken left leg.
