Location: Jobsite; On or about 03/21/2025, the employer did not instruct employees in the recognition and avoidance of unsafe conditions and the regulations applicable to the job site, specifically the employee as permitted to utilize a Makita GA7021 7" Angle Grinder [Serial #: 807244A] against the manufacturer's instructions by using the equipment as a masonry saw to cut through the concrete floor and had positioned themself or a portion of their body in the path of kickback or kickout of the equipment while in operation.
