29 CFR 1910.178(a)(4): Capacity, operation, and maintenance instruction plates, tags or decals of the powered industrial truck were not changed accordingly. (a) In the fabrication shop - On or about March 26, 2025, the employer exposed employees to struck-by hazards, in that, the Toyota forklift (Model 8FGC70U, serial number 11217) did not have an updated capacity plate, tag or decal for the 10-foot Abaco bundle slab loader (Model ACBSL5T, serial number 02201221006).
