Lacko Poured Walls, LLC. at a worksite located at 3885 Mason Lane, Grove City, Ohio 43123: On or about March 26, 2025, the employer did not ensure employees were protected from a fall up to approximately three feet and caught-between hazards while lifting employees with the bucket of a Caterpillar 320 hydraulic excavator to place a cap on a newly installed pipe riser.  a. Lacko Poured Walls, LLC. at a worksite located at 3885 Mason Lane, Grove City, Ohio 43123: On or about March 26, 2025, the employer did not ensure employees were protected from a fall up to approximately 14-feet and caught-between hazards while lifting employees with the bucket of a Caterpillar 320 hydraulic excavator to brace a dewatering hose for removal from an open trench.

Lacko Poured Walls LLC at a worksite located at 3885 Mason Lane, Grove City, OH 43123: On or about March 26, 2025, and at times prior, the employer did not ensure that a safe means of egress was located in trenches that were more than 4 feet deep. Employees performing sewer tie-in work in a trench over ten feet deep did not have a ladder, ramp, or other means for the employees to egress. The condition exposed employees to struck-by/caught-in hazards.  b. Lacko Poured Walls LLC at a worksite located at 3885 Mason Lane, Grove City, OH 43123: On or about March 26, 2025, and at times prior, the employer did not ensure that a safe means of egress was located in an excavation that was more than 4 feet deep. Employees measuring grade and marking footers from inside a basement excavation over seven feet deep did not have a ladder, ramp, or other means for the employees to egress.   Lacko Poured Walls was previously cited for a violation of this Occupational Safety and Health standard 29 CFR  1926.651(c)(2), which was contained in OSHA inspection number 1532177, citation number 1, item number 1 and was affirmed as a final order on July 1, 2021, with respect to a workplace located at 155 Beachgate Drive, Granville, OH 43023.

An inspection shall be conducted by the competent person prior to the start of work and as needed throughout the shift. Inspections shall also be made after every rainstorm or other hazard increasing occurrence. These inspections are only required when employee exposure can be reasonably anticipated.  a. Lacko Poured Walls, LLC at a worksite located at 3885 Mason Lane, Grove City, OH 43123: On or about March 26, 2025, and at times prior,  the employer did not ensure that an inspection of a basement/foundation excavation was performed by a competent person to ensure that a compliant protective system was in place prior to permitting employees to work in the excavation.  b. Lacko Poured Walls, LLC at a worksite located at 3885 Mason Lane, Grove City, OH 43123: On or about March 26, 2025,  the employer did not ensure that an inspection of the trench was performed by a competent person to ensure that a compliant protective system was in place prior to permitting employees to work in the trench to perform a residential sanitary tie-in installation.  Lacko Poured Walls, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.651(k)(1), which was contained in OSHA inspection number 1579978, citation number 1, item number 2 and was affirmed as a final order on June 1, 2022, with respect to a workplace located at 234 Carrowmoore Drive in Pickerington, Ohio.

Employees worked in and traversed multiple sections of a trench without any protective system in place. The condition exposed employees to struck-by and caught-in hazards.  b. Lacko Poured Walls, LLC at a worksite located at 3885 Mason Lane, Grove City, OH 43123: On or about March 26, 2025, and at times prior, the employer did not ensure employees were protected from cave-ins by a compliant protective system while performing work in a basement excavation that was over seven feet in depth. Employees were in an excavation marking/preparing the excavation for footings while heavy equipment(an excavator/dump trucks) was operating in the area, and spoils were not set back at least two feet. The condition exposed employees to struck-by and caught-in hazards.  Lacko Poured Walls, LLC was previously cited for a violation of this Occupational Safety and Health standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1579978, citation number 1, item number 3 and was affirmed as a final order on June 1, 2022, with respect to a workplace located at 234 Carrowmoore Drive in Pickerington, Ohio
