Location: 77 Charlton St in New York, NY. a) On or about 3/26/2025, the employer did not ensure that the rope descent systems had proper tiebacks and anchorages to the building. The rope system was tied off to the fence posts on the edge of the roof as anchorage.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): Location: 77 Charlton St in New York, NY. a) On or about 3/26/2025, the employer did not ensure that employees were protected from falling by using fall protection such as, but not limited to, personal fall arrest systems, safety nets, and/or guardrails. Employees working as window washers were not protected while crossing over the fence at the edge of the roof and were exposed to falls approximately 40 feet (4 stories).
