All splices and joints and the free ends of conductors were not covered with an insulation equivalent to that of the conductors or with an insulating device identified for the purpose:  On or about 03/26/2025, at 1160 West 13th Street, Unit 10, Riviera Beach, Florida, an extension cord was spliced with electrical tape, thus exposing employees to potential electrical shock hazards.
