29 CFR 1915.73(d):When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2). Barge - On or about, March 25,2025 and at times prior thereto, employees performing dry dock construction on top of the floating barge were exposed to a fall, of approximately 12', from the deck of the barge to the barge below in that the edges were not guarded or fall protection utilized.

29 CFR 1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: Barge: On or about March 25, 2025, and at times prior thereto, employees utilizing powered hand tools, were exposed to electrical shock in that the machine was not connected so that strain relief was provided. The insulated jacket near the plug had pulled away exposing the electrodes.
