29 CFR 1926.416(a)(1): The employer permitted employees to work in close proximity to energized, unguarded, and ungrounded electric power circuits that the employee could contact in the course of their work: a) Worksite - On or about March 26, 2025, an employee working from an aerial lift (JLG - 1350SJP, Number on aerial lift: 416505) that was not insulated was exposed to electrocution hazards due to cutting the metal beam (leg) of the water tower within 10 feet of electric power lines (13.2kV) that were not turned off and temporarily relocated before the demolition work of taking down a water tower.

29 CFR 1926.850(c): All electric, gas, water, steam, sewer, and other service lines were not shut off, capped, or otherwise controlled, outside the building line before demolition work was started, and the affected utility companies were not notified: a) Worksite - On or about March 26, 2025, an employee working from an aerial lift (JLG - 1350SJP, Number on aerial lift: 416505) that was not insulated was exposed to electrocution hazards due to cutting the metal beam (leg) of the water tower within 10 feet of electric power lines (13.2kV) that were not turned off before the demolition work of taking down a water tower started, and the affected utility company was not notified.
