29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Worksite, 118 Main St, Schaghticoke, NY, Roof - On or about March 27 ,2025, employees were performing roofing repair work on the roof of a residential building approximately 20 feet above the ground and did not have fall protection. Employees did not have anchor points, lifeline, harnesses and lanyards.

29 CFR 1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use: a) Worksite, 118 Main St, Schaghticoke, NY, Roof - On or about March 27 ,2025, employees were accessing the roof of a residential building approximately 10 feet above the first level roof using a aluminum extension ladder with a broken right-side rail. The ladder was not inspected for visible defects by a competent person.
