Listed, labeled, or certified equipment shall be installed and used in accordance with instructions included in the listing, labeling, or certification. a) On or about March 27, 2025, the employer did not ensure flexible extension cords were used in accordance with the manufacturer's instructions, in that two flexible extension cords were connected together to supply power to a Shop Vac.

29 CFR 1926.404(f)(6):Grounding path. The path to ground from circuits, equipment, and enclosures shall be permanent and continuous. a) On or about March 27, 2025, the employer failed to ensure that the path to ground from electrical circuits to flexible extension cords located in the front of the building was permanent and continuous. Specifically, the pigtail triple extension cord powering the shop vac was missing the grounding pin thus exposing employees to electrical shock hazards while using the shop vac.

29 CFR 1910.134(k)(6):The basic advisory information on respirators, as presented in Appendix D of this section, shall be provided by the employer in any written or oral format, to employees who wear respirators when such use is not required by this section or by the employer. a) On or about January 6, 2025, the employer did not provide all of the information contained in Appendix D to include cleaning and care, limitations, and use of the respirator in atmospheres containing contaminants the respirator is not designed to protect against for employees voluntarily wearing 3M 8210V or Milwaukee 50-73-400 TC-84A-8142 N95 filtering facepiece respirators while performing demolition activities.
