29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a. At a temporary, new construction worksite of a commercial building, located at or near, 11341 Normandy Boulevard in Jacksonville, Florida 32221: On or about March 26, 2025, the employer exposed employees  to fall hazards, in that fall protection was not utilized by employees while performing roofing work.

29 CFR  1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not  independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 KN) per employee attached:  a. At a temporary, new commercial building construction worksite, located at or near, 11341 Normandy Boulevard in Jacksonville, Florida 32221: On or about March 26, 2025, the employer exposed employees to fall hazards in that employee lanyards were anchored to supports that were being used to suspend the working platform.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a.   At a temporary, new commercial building construction worksite, located at or near, 11341 Normandy Boulevard in Jacksonville, Florida 32221: On or about March 26, 2025, the employer exposed employees to fall hazards in that the portable extension ladders side rails were not extended a least three feet above the upper landing surface to which the ladder is used to gain access; nor did  it have grasping device, such as a grabrail, to assist employee in mounting and dismounting the ladder.
