On or about March 26, 2025, the employer did not ensure employees were protected from fall hazards while they installed water proofing materials on an buildings eave in that the employer used an A-frame ladder that was wire tied to a scaffolding for employees to gain access to the eave instead of a ladder that was specifically designed for use with the type of scaffold used. The employer exposed employees to a 13' fall height from an A-frame ladder that was wire tied to a scaffolding system.

Guardrail systems shall be installed before the scaffold is released for use by employees other than erection/dismantling crews. a.) On or about March 26, 2025, the employer did not ensure employees were protected from fall hazards while they installed water proofing material on a building walls in that the employer did not install a guardrail system on all sides of the scaffolding system exposing employees to a 13' fall hazard.
