29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment (PPE) that would protect the affected employee from the hazards identified in the hazard assessment: Main Shop (Plating Area): On or about April 22, 2025, employer did not include in the employer's written PPE assessment the selection and use personal protective equipment that would protect employees' arms and feet from splash hazards from hazardous chemicals including, but not limited to, hexavalent chromium compounds during plating operations including, but not limited to, chromate plating of metal (aluminum) products (chemical processing of customer product) exposing employees to hazardous chemicals including, but not limited to, hexavalent chromium (Aurous 19).

29 CFR 1910.1026(l)(1)(iii): Employer did not include chromium (VI) in the hazard communication program established to comply with the HCS (� 1910.1200). Employer did not ensure that each employee had access to labels on containers of chromium (VI) and to safety data sheets, and was trained in accordance with the requirements of HCS and paragraph (l)(2) of this section: Main Shop (Plating Area): On or about April 22, 2025, employer did not ensure that employees who were exposed to hexavalent chromium during chromate plating operations had access to the labels for Process Tank #5A Hex Chromate Class 1A and Process Tank #5B Hex Chromate Class 3 that included at least skin sensitization hazards as well as other information required by paragraph (l)(1)(ii) of this section.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical: Main Shop (Plating Area): Employer did not ensure that each open surface process tank of hazardous chemical(s) was labeled, tagged or marked with the product identifier, at least general information regarding the hazards of the chemicals and readily accessible in the work area throughout each work shift for employees who performed anodize, chromate, passivation, and hard-coat plating of metal (aluminum) parts. Hazardous chemicals in the open surface process tanks included, but were not limited to, sodium hydroxide, nitric acid, sulfuric acid, hexavalent chromium, and trivalent chromium.
