On or about March 28, 2025, during the installation of roofing material at the job site located at 310 2ND ST SW, Massillon Ohio 44646; during the installation of roofing material, the employer did not ensure employees working on the lower roof were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 18.3 feet.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about March 28, 2025, during the installation of roofing material at the job site located at 310 2ND ST NW, Massillon Ohio 44646; the employer did not ensure employees working on a roof of a single-family home were protected from a falls from two different roof heights. The upper roof fall height was 18.3 feet while the lower roof had a fall height of 8.4 feet above the ground. The supervisor was on site and working along side the exposed employees.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about March 28, 2025, during the installation of roofing material at the job site located at 310 2ND ST SW, Massillon Ohio 44646; during the installation of roofing material on a residential structure, the employer did not ensure that employees utilized portable ladders that were extended 3 feet above the upper landing surface. The foreman was on site and utilized the portable ladder to gain access/egress to the working surface of the roof which was 18.3 feet above the ground.
