29 CFR 1926.25(a):During the course of construction, alteration, or repairs, debris was not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: (a) On or about March 27, 2025, and at times prior, Unlimited Framing, Inc., as a controlling employer did not ensure that work areas and passageways were kept cleared of any construction debris. A subcontractor employee sustained serious injuries when he tripped over construction debris and fell through the wall opening approximately 21 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) On or about March 27, 2025, and at times prior, Unlimited Framing, Inc., as a controlling employer did not ensure that subcontractor employees conducting cleaning operations adjacent to a wall opening were protected from fall hazards in excess of 6 feet. A subcontractor employee sustained serious injuries when he tripped over construction debris and fell through the wall opening approximately 21 feet.
