29 CFR 1910.140(c)(10)(v): The employer did not ensure that fall protection system snaphooks and carabiners are not connected to any object that is incompatibly shaped or dimensioned in relation to the snaphook or carabiner such that unintentional disengagement could occur when the connected object depresses the snaphook or carabiner gate, allowing the components to separate, unless they are designed for such connections: a) On or about March 23, 2025, at the jobsite, the employer did not ensure carabiners used during climbing operation to remove the lighting systems was compatible with structure to prevent unintentional disengagement.
