29 CFR 1926.651(i)(3): Sidewalks, pavements, or appurtenant structures had been undermined and a support system or another method was not provided to protect employees from the possible collapse of such structures: Migeon Ave, Torrington, CT 06790: On or about March 28, 2025, the employer had not ensured that a concrete sidewalk slab had additional support systems to prevent the slab from sliding into the excavation and striking an employee's leg.

29 CFR 1926.651(k)(1): An inspection of the excavations, the adjacent areas, and protective systems was not made after every rainstorm or other hazard increasing occurrence. Migeon Ave, Torrington, CT 06790: On or about March 28, 2025, no inspection was conducted when an excavation that started in the roadway was extended to include a section through the sidewalk adjacent to the road. Removal of soil in the extension undermined the sidewalk slab causing it to slide into the excavation, injuring an employee in the trench.
