29 CFR 1926.20(b)(2): The employer did not initiate and maintain frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a)  On or about April 1, 2025,  at a jobsite located at W273N1531 Lakefield Dr. PEWAUKEE, WI 53072,  a competent person was on site but did not perform frequent and regular inspections of the job site to ensure that hazardous conditions were identified and corrected.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a) On or about April 1, 2025, at a jobsite located at W273N1531 Lakefield Dr., Pewaukee, WI 53072, employees conducting roofing activities to include paper and shingle installation on an 8:12 pitch roof of the structure were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 10-feet 10-inches onto concrete, grass, and building materials.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access, or were not secured:  (a) On or about April 1, 2025, at a jobsite located at W273N1531 Lakefield Dr, Pewaukee, WI 53072, the portable telescoping ladders being used to access the upper landing area or roof of the structure did not extend at least three feet above the upper landing area and were not secured. This exposed employees accessing and egressing to and from the roof, to a fall hazard of approximately  10-feet 10-inches onto grass, building materials, and concrete .
