29 CFR  1910.106(e)(3)(iv)(a): Emergency drainage systems were not provided to direct flammable liquid leakage and fire protection water to a safe location:   a) Storage Room:   Category 2 and Category 3 flammable chemicals (e.g., isopropyl alcohol, xylene, used in unit physical operation-type manufacturing were stored in 55-gallon drums along with 5-gallon containers (e.g., drums) containing  Category 3 flammable finished products which were manufactured in unit physical operations and emergency drainage systems were not provided; on or about 3/31/25.  b) Storage Rack System - IBC Storage Area:   Category 3 flammable finished products (e.g., STS-21 Plus) which were manufactured in unit physical operations were stored in two 275-gallon intermediate bulk containers and emergency drainage systems were not provided; on or about 3/31/25.  In addition to abatement certification, the employer is required to submit abatement documentation for this item.  Failure to comply will result in additional penalty of $ 1,000.00 as per 29 CFR 1903.19

29 CFR  1910.106(e)(6)(ii):  Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), were dispensed into containers without the nozzle and container being electrically interconnected:  a) Plant Floor near the Dye Storage Area:    Employees were dispensing isopropyl alcohol (Category 2 - Flammable Liquid) from a 55 gallon drum into a plastic pail without the nozzle and the container being electrically interconnected; on or about 3/31/25.  In addition to abatement certification, the employer is required to submit abatement documentation for this item.  Failure to comply will result in additional penalty of $ 1,000.00 as per 29 CFR 1903.19

29 CFR  1910.106(h)(4)(iv)(a):  Equipment was not designed and arranged to prevent the unintentional escape of liquids and vapors and to minimize the quantity escaping in the event of accidental release:  a) Plant Floor - Near Dye Storage Area:   On or about 3/31/25, isopropyl alcohol (Category 2 - Flammable Liquid) was being dispensed by gravity from a 55-gallon drum into a 5-gallon pail using equipment that was not designed to prevent the unintentional escape of flammable liquids and vapors in the event of an accidental release (e.g., approved self-closing valve or equivalent).  In addition to abatement certification, the employer is required to submit abatement documentation for this item.  Failure to comply will result in additional penalty of $ 1,000.00 as per 29 CFR 1903.19

29 CFR  1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check:  a) Outside the Storage Room for flammable chemicals:   On or about 9/25/25, a carbon dioxide fire extinguisher as last inspected in 2021 and had not been subject to an annual maintenance check.  b) Passageway between Office Area and Plant Floor:   On or about 9/25/25, a carbon dioxide fire extinguisher was last inspected in 2021 and had not been subject to an annual maintenance check.  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19, failure to comply will result in additional penalty of $ 1,000.00.
