29 CFR  1910.95(c)(1):The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.9(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:  A) 74 Liberty St. Metuchen, NJ 08840.  On or about April 1, 2025 and thereafter, the employer failed to have a Hearing Conservation Program, including providing an annual audiometric testing and training  to employee(s) exposed to excess noise throughout the facility.    OSHA conducted personal exposure monitoring for noise on May 2, 2025, while employees were conducting normal daily activities for three employees in the pallet Dismantling Room and two employees in the Pallet Assembly Room.   In the Pallet Dismantling Room personal monitoring measured exposures to noise at 265%, 208%  and 100% Dose; equivalent to 8-hour Time-weighted averages (TWA) of  97dBA, 95 dBA and 90 dBA respectively.   In the Pallet Assembly Room personal monitoring measured exposures to noise at 179% and 104% Dose; equivalent to 8-hour Time-weighted averages (TWA) of 94 dBA and 90 dBA respectively.
