29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment, or operations, which were out of the ordinary, unexpected, or not readily apparent, and tags were not used until such time as the identified hazard was eliminated or the hazardous operation was completed: (a) Processing, Raw Honey Area, Pittsburgh Automotive Hoist - The hoist cable became twisted causing the hoist to not respond to the pendant controller. Prior to leaving the area, an employee did not attach a tag to the equipment to warn other employees not to use the equipment due to the malfunctioning condition, on or about March 17, 2025. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
