29 CFR 1910.145(f)(3): Tags were not used as a means to prevent accidental injury or illness to employees who were exposed to hazardous or potentially hazardous conditions, equipment, or operations, which were out of the ordinary, unexpected, or not readily apparent, and tags were not used until such time as the identified hazard was eliminated or the hazardous operation was completed:  (a) Processing, Raw Honey Area, Pittsburgh Automotive Hoist - The hoist cable became twisted causing the hoist to not respond to the pendant controller.  Prior to leaving the area, an employee did not attach a tag to the equipment to warn other employees not to use the equipment due to the malfunctioning condition, on or about March 17, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft ends did not present a smooth edge and end and projected more than one half the diameter of the shaft:  (b) Bottling Room, Line #3, Rear Side - The horizontal shaft end, 26 inches above the floor, on the belt conveyor after the handle applicator, did not present a smooth edge due to having set screws, and this shaft end did not have guarding, on or about April 1, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  (a) Bottling Room, Line #3, Rear Side, near the Labeler - Two different multi-receptacle metal knockout boxes, each installed as a pendant on a flexible cord, supplied power to cord and plug connected equipment.  This use did not follow the listing, labeling, and certification for receptacles, which require using non-metallic outlet boxes and non-metallic strain relief, on or about April 1, 2025.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation, or the loss of an eye:  (a) 2220 Dutch Gold Drive, Lancaster, Pennsylvania - An employee suffered an amputation of the tip of the left middle finger on March 17, 2025 after getting the fingertip caught in a hoist.  The employer did not report this amputation to OSHA within 24 hours.  No abatement certification or documentation required.
