29 CFR  1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle:   a.) Facility, 26 Rich Hill Road, Cheswick, PA 15024: On or about March 28, 2025, Gate 1, dehacker machine latch on the safety interlock device  was bypassed on the equipment effectively eliminating the guarding mechanism which was intended to control the hazard at the pinch point of operation.  An employee sustained serious injuries while being pinned between the de-hacker machine and a stack of bricks.
