29 CFR 1926.451(b)(5)(ii): Each platform greater than 10 feet in length shall not extend over its support more than 18 inches (46 cm), unless it is designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end.  Location - Worksite:  On or about 4/1/25, a scaffold platform that was longer than 10 feet extended 3 feet beyond its support.  Mirands Construction, Inc. was previously cited for a violation of this occupational safety and health standard 1926.451(b)(5)(ii) which was contained in OSHA inspection number 1526369, citation number 1, item number 1B and was affirmed as a final order on 8/18/21, with respect to a workplace located at 64 Raddin Street, Lynn, MA.

(a) Location - Worksite:   On or about 4/1/25, an employee was exposed to 12-foot fall hazards while installing shingles on a residential roof from a ladder jack scaffold without fall protection.  (b) Location - Worksite:  On or about 4/1/25, an employee was exposed to 21-foot fall hazards while installing shingles on a residential roof from an unapproved scaffold plank raised by an all-terrain lift without adequate fall protection.    Mirands Construction, Inc. was previously cited for a violation of this occupational safety and health standard 1926.451(g)(1) which was contained in OSHA inspection number 1526369, citation number 1, item number 3 and was affirmed as a final order on 8/18/21, with respect to a workplace located at 64 Raddin Street, Lynn, MA.
