29 CFR 1910.28(b)(2)(ii): The employer did not ensure that when any portion of a guardrail system, gate, or chains is removed, and an employee must lean through or over the edge of the access opening to facilitate hoisting, the employee is protected from falling by a personal fall arrest system.  On or about April 3, 2025, on the 3.60 Rotomolding machine operators were exposed to a fall hazard when a portion of the guardrail system was opened to retrieve and remove materials from the Rotoline Mobile Platform. Employees were not provided a personal fall arrest harness and were exposed to serious injury from impact to the lower level.
