29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about April 2, 2025, employees working on and around the Lever LS7000 Slitting Machine (Model No. LS7000 & Serial No. LS7040A-110) and the employer did not ensure the slitting machine was properly guarded, in-that the interlocks to access panels were overridden. This exposed employees to amputation hazards. (b) On or about April 2, 2025, employees working on and around the Lever LS7000 Slitting Machine (Model No. LSM7000 & Serial No. LSM7040A-116) and the employer did not ensure the slitting machine was properly guarded, in-that interlocks to access panels were overridden. This exposed employees to amputation hazards.
