On or about March 25, 2025,on the roof of residential building, employees were not in use of fall protection while performing roofing work activities at heights greater than six (6) feet.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about March 25, 2025, on the norths side of the residential building, employees performing roofing work activities were using a portable extension ladder to access the roof and the side rails did not extend at least three (3) feet above the landing.
