29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: a) Warehouse Area - On April 3, 2025, the employer did not ensure that the Komatsu forklift model #FG25ST-14, serial #581130A, powered industrial truck with brakes that do not completely stop immediately, had damaged tires, and a front light cover that was cracked and damaged, was withdrawn from service until restored to safe operating condition. b) Warehouse Area - On April 3, 2025, the employer did not ensure that the Crown forklift model #RRS200, serial #1A333318, powered industrial truck with a damaged back rest in that the uprights were bent was withdrawn from service until restored to safe operating condition. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
