29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.) At the worksite, located at 16127 Rutland Rd Parrish, Fl 34219; On or about 4/3/25, the employer exposed employee to struck by hazards in that head protection was not provided for employees working on the ground level as roofing operations were conducted above approximately 18 ft in height.

At the worksite, located at 16127 Rutland Rd Parrish, Fl 34219; On or about 4/3/25, the employer exposed employee to fall hazards approximately 9 ft & 20 ft in height with extension ladders not extended for employees 3ft above the work surface while conducting roofing operations on a residential home.
