29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a) Around the building - On or about April 3, 2025, employees walking next to the building, while picking up trash that had fallen off the roof, did not use head protection, exposing them to struck-by hazards.

29 CFR  1926.416(a)(1): Employee(s) were permitted to work in proximity to an electric power circuit that the employee could contact in the course of work and was not protected against electric shock by de-energizing the circuit and grounding it or by guarding it effectively by insulation or other means:  (a) Southwest corner of roof - On or about April 3, 2025, the employer instructed his employees to remove and replace the roofing shingles while working next to the energized power line to the house without any type of electrical shock protection exposing employees to electrical shock hazards.

29 CFR  1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  (a) Top of roof - On or about April 3, 2025, the employer instructed his employees to remove the old roofing shingles from the roof the single story building then install new shingles without providing any means of fall protection exposing employees to fall hazards.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) East side of roof - On or about April 3, 2025, the employer provided a little giant step ladder for his employees to use to gain access to the roof that did not extend 3 feet past the edge of the roof exposing employees to fall hazards.
