29CFR 1926.20(b)(2) The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). On or about April 3, 2025, employees engaged in residential utility repair, located at 3521 Center St., Omaha, NE 68105, was exposed to cave-in hazards in that the employer had an employee working in an trench that was in excess of 5-feet with inadequate protective systems. This excavation included benching in Type C soil that was less than 1 1/2 to 1 in ratio as required by the standard.
