Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  One compressed gas cylinder of Argon was freestanding and was not secured by any device to prevent the cylinder from falling, rolling, or getting knocked over.

29 CFR  1910.215(a)(4):Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  a.)	Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Serial # 18060140)  with the work rest on the right side of the grinder at approximately 5/8" from the grinding wheel/stone.  The work rest had not been adjusted as the wheel was worn.  b.)	Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Serial # 18060140)  with the work rest on the left side of the grinder at approximately 1/2" from the grinding wheel/stone.  The work rest had not been adjusted as the wheel was worn.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Model: IBG8VS; Serial # 18060140)  with the tongue guard on the right side of the grinder located at approximately 1 and 3/8" inches from the grinding wheel/stone.  The tongue guard had not been adjusted as the wheel was worn.  b.)	Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Model: IBG8VS; Serial # 18060140)  with the tongue guard on the left side of the grinder located at approximately 1 and 3/4" inches from the grinding wheel/stone. 29 CFR  1910.215(d)(3): The contact surface(s) of wheel(s), blotter(s) or flange(s) on grinding machine(s) were not flat and free of foreign matter:  a.)	Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Model: IBG8VS; Serial # 18060140)  for grinding tungsten electrodes for TIG welding operations.  Employees were potentially exposed to a struck-by hazard in that, the right grinding wheel had a groove worn into the middle of the grinding wheel.  The surface of the wheel on the right side of the industrial grinder was not maintained in a flat condition.  b.)	Flowline Divison-Ezeflow USA Inc., Welding department area, on or about April 3rd, 2025 and times prior thereto:  Employees utilized a Jet industrial grinder (Model: IBG8VS; Serial # 18060140)  for grinding tungsten electrodes for TIG welding operations.  Employees were potentially exposed to a struck-by hazard in that, the left grinding wheel had a groove worn into the right side of the of the grinding wheel.  The surface of the wheel on the left side of the industrial grinder was not maintained in a flat condition.
