29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: On or about March 31, 2025, it was documented that employees performing concrete masonry at Casey's a construction site located at 2700 Plum Creek Parkway, Lexington, Nebraska were exposed to shock hazards in that the employer did not provide site hazard information to employees including but not limited to the use of bull floats with at least one 72 inch by 1 3/4 inch Kraft Tool Company CC289SB Swaged Button Handle-Blue aluminum and three other metal extensions in immediate proximity to energized 7,900 volt overhead electrical lines approximately 25 feet phase to ground.

29 CFR 1926.416(a)(1): The employer permitted employee(s) to work in such proximity to an electric power circuit that the employee(s) could contact the electric power circuit in the course of work: On or about March 31, 2025, it was documented that employees performing concrete masonry at Casey's a construction site located at 2700 Plum Creek Parkway, Lexington, Nebraska were exposed to shock hazards in that employees used a bull float with at least one 72 inch by 1 3/4 inch Kraft Tool Company CC289SB Swaged Button Handle-Blue aluminum and three other metal extensions in immediate proximity to energized 7,900 volt overhead electrical lines approximately 25 feet phase to ground.

29 CFR 1926.702(h): Bull float handles, used where they might contact energized electrical conductors, were not constructed of nonconductive material or insulated with a nonconductive sheath whose electrical and mechanical characteristics provided the equivalent protection of a handle constructed of nonconductive material: On or about March 31, 2025, it was documented that employees performing concrete masonry at Casey's a construction site located at 2700 Plum Creek Parkway, Lexington, Nebraska were exposed to shock hazards in that employees used at least one 72 inch by 1 3/4 inch Kraft Tool Company CC289SB Swaged Button Handle-Blue aluminum and three other metal extensions in immediate proximity to 7,900 volt overhead electrical lines approximately 25 feet phase to ground resulting in the electrical shock of an employee.
