29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. On or about April 3, 2025, two employees were working at heights between approximately 11 feet to 15 feet standing on a makeshift a scaffold lifted in the air by a Sky Trak Model 6036 all-terrain forklift, that was not specifically designed to do so.

29 CFR 1926.451(c)(2)(v): Fork-lifts shall not be used to support scaffold platforms unless the entire platform is attached to the fork and the fork-lift is not moved horizontally while the platform is occupied. On or about April 3, 2025, two employees were working at heights between approximately 11 feet to 15 feet standing on a makeshift a scaffold lifted in the air by a Sky Trak Model 6036 all-terrain forklift, that was not secured to the forklift.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. On or about April 3, 2025, two employees were working at heights between approximately 11 feet to 15 feet standing on a makeshift a scaffold and were not protected from falling to that lower level by fall protection.
