29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): a) Trench located at the northwest corner of residential home: On or about April 3, 2025, and at times prior, employees were exposed to cave-in hazards while performing foundation lifting operations of a residential home within an unprotected trench in excess of 5 feet in depth. The employer failed to ensure employees were protected from cave-ins while digging inside a trench, in that there was no means of protection provided. b) Trench located at the front door entrance of residential home: On or about April 3, 2025, and at times prior, employees were exposed to cave-in hazards while performing foundation lifting operations of a residential home within an unprotected trench in excess of 5 feet in depth. The employer failed to ensure employees were protected from cave-ins while working inside a trench, in that the north side of the trench was not protected.
