29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: a) Clearwater Granite Shop: On or about April 2, 2025, and at time prior, employees were exposed to respirable crystalline silica while wearing tight fitting 3M half mask elastomeric respirators with particulate cartridges to perform cutting and grinding on stone countertop slabs. The employer failed to ensure that employees were medically evaluated prior to respirator use. b) Clearwater Granite Shop: On or about April 2, 2025, and at time prior, employees were exposed to respirable crystalline silica while wearing tight fitting 3M half mask elastomeric respirators with particulate cartridges to perform cutting and grinding on stone countertop slabs. The employer failed to ensure employer were fit tested prior to respirator use. c) Clearwater Granite Shop: On or about April 2, 2025, and at time prior, employees were exposed to respirable crystalline silica while wearing tight fitting 3M half mask elastomeric respirators with particulate cartridges to perform cutting and grinding on stone countertop slabs. The employer failed to ensure a respiratory maintenance program was developed. d) Clearwater Granite Shop: On or about April 2, 2025, and at time prior, employees were exposed to respirable crystalline silica while wearing tight fitting 3M half mask elastomeric respirators with particulate cartridges to perform cutting and grinding on stone countertop slabs. The employer failed to ensure that employees were trained on respirator use and limitations prior to use.
