29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system; and/or on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   (a) Northwest slope of rooftop; On or about April 4, 2025, fall protection systems were not used during reroofing operations, while existing roofing materials were being manually removed from a 4/12 sloped roof that had a peak height of 11 feet, 1 inch and the eave height was 8 feet, 10 inches above the ground.

29 CFR  1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Northwest side of roof; On or about April 4, 2025, the siderails of a Keller brand, 10-foot fiberglass stepladder used to get on and off the roof did not extend at least 3 feet above the roofs eave, where they accessed the roof, which was 8 foot, 10 inches above ground level.

29 CFR  1926.1053(b)(15): Ladders were not inspected by a competent person for visible defects on a periodic basis and after an occurrence that could affect their safe use:   (a) Northwest side of roof; On or about April 4, 2025, a Keller brand, 10-foot fiberglass stepladder used to get on and off the roof and support a work plank was not inspected for visible defects.  The ladder had a bent rear horizontal brace and two locations on the back siderails were cracked through where the rivets attached rear horizontal braces to the rails.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:   (a) Northwest side of roof; On or about April 4, 2025, a damaged Keller brand, 10-foot fiberglass stepladder was not immediately marked as defective and/or removed from service.  The ladder was used to get on and off the roof and support a work plank while it had a bent rear horizontal brace and two locations on the back siderails were cracked through where the rivets attached rear horizontal braces to the rails.
